What the federal rule says
Each entry is condensed from the paragraph cited; the paragraph itself is the rule and the link opens it.
29 CFR 1926.1153(e)(1): Where respiratory protection is required by this section, the employer must provide each employee an appropriate respirator that complies with the requirements of this paragraph and 29 CFR 1910.134. Respiratory protection is required: where specified by Table 1 of paragraph (c) of this section; or for tasks not listed in Table 1, or where the employer does not fully and properly implement the engineering controls, work practices, and respiratory protection described in Table 1: where exposures exceed the PEL during periods necessary to install or implement feasible engineering and work practice controls; where exposures exceed the PEL during tasks, such as certain maintenance and repair tasks, for which engineering and work practice controls are not feasible; and during tasks for which an employer has implemented all feasible engineering and work practice controls and such controls are not sufficient to reduce exposures to or below the PEL.
29 CFR 1926.1153 Table 1 (example rows): Handheld power saws (any blade diameter) with integrated water delivery: when used outdoors, no respirator for a shift of 4 hours or less and APF 10 for more than 4 hours; when used indoors or in an enclosed area, APF 10 for either shift length. Stationary masonry saws with integrated water delivery: none.
29 CFR 1910.1053(g)(1) (general industry): Where respiratory protection is required by this section, the employer must provide each employee an appropriate respirator that complies with the requirements of this paragraph and 29 CFR 1910.134. Respiratory protection is required: where exposures exceed the PEL during periods necessary to install or implement feasible engineering and work practice controls; where exposures exceed the PEL during tasks, such as certain maintenance and repair tasks, for which engineering and work practice controls are not feasible; during tasks for which an employer has implemented all feasible engineering and work practice controls and such controls are not sufficient to reduce exposures to or below the PEL; and during periods when the employee is in a regulated area.

Respirators for Silica Dust questions
Is an N95 enough for silica? The silica standards set the respirator by APF, not by filter class: Table 1 of 1926.1153 specifies APF 10 (a half mask air-purifying respirator, which includes a filtering facepiece) or APF 25 for the listed tasks, and other tasks are selected under 1910.134 against the exposure. The makers' data gives N95 as 95% minimum efficiency against non-oil aerosols and P100 as 99.97% against all particulates.
What is the silica PEL? 50 μg/m³ as an 8-hour TWA under 29 CFR 1926.1153 and 1910.1053 and under Cal/OSHA Title 8 §1532.3 and §5204; the action level is 25 μg/m³.
